Online appointment requests must be made a minimum of 72 hours in advance of requested appointment time. Please note that this form is only for requests of routine appointments.  We try to accommodate your preferred date and time, but your appointment is only booked when you receive an email confirmation from Kelley Animal Hospital. If you need to schedule a surgery, dental procedure, or other non-routine appointment, please give us a call and we’ll be happy to help you.
